A review of the facility’s policy titled “Evaluation Process,” dated December 19, 2022, indicated that the facility is to review employee work performance with a formal written evaluation annually, considering factors such as job performance, achievement of preset goals, attendance records, and adherence to workplace policies. The absence of annual performance evaluations for the three CNAs was therefore not in accordance with the facility’s own written policy and represented a failure to follow established procedures for monitoring and documenting staff performance and competency.

The DON/Designee will complete annual evaluations for Nurse Aide on an annual basis. The Administrator will provide education to the facility DON/Designee on the importance of completing Annual Evaluations on the employee in the facility. These evaluations will include Core Values and Objectives, of Quality of Work, Attendance and Punctuality, Communication Skills, Judgement and Decision-making skills Cooperation and Teamwork and Knowledge of position. Audits will be completed by the HR Director/Designee on 10% of Nurse Aide employees each month that the Employee Annual Evaluation has been completed. The results of these audits will be reviewed by the QAPI committee for further recommendations
