Summary

The facility failed to ensure that residents with hearing and vision needs were provided access to appropriate ancillary services. For Residents #105 and #106, both of whom were cognitively intact and had documented hearing deficits, the record showed hearing-related concerns and care plan interventions, but no audiology appointments were scheduled. Resident #106 reported that he/she and a spouse were sharing hearing aids because both devices were broken and they had not been able to see the audiologist since July 2025. Resident #105 showed the surveyor a hearing aid that was broken in two pieces and stated that the device had been broken for some time and that he/she did not understand why audiology care was taking so long. Resident #106’s record showed a consent for ancillary mobile health services requesting audiology, dental, eye care, and podiatry, and progress notes documented decreased hearing and hearing impairment. Resident #105’s record also showed a consent for ancillary mobile health services requesting audiology, dental, eye care, and podiatry, and progress notes documented that the resident was not hearing well on multiple occasions. Despite these documented concerns, physician orders did not include audiology consults for either resident, and the ancillary services consultant stated that neither resident had audiology appointments scheduled. The social worker acknowledged awareness of the hearing aid concerns and stated there was no system in place to track ancillary service needs and no further follow-up after emails were sent. The facility also failed to arrange optometry services for Resident #29, who had diagnoses including diabetes mellitus and optic neuritis and was cognitively intact. The resident signed a request for eye care, and physician and NP progress notes repeatedly stated that the resident would be referred to optometry, but the consultant eye care group schedule did not show the resident as scheduled between April 2025 and February 2026. The care plan did not identify vision as a focus area. Survey observations showed the resident not wearing glasses on multiple occasions, and the resident stated that the glasses were no good and that a new pair might help him/her see more clearly. The consultant eye care group confirmed the resident was not enrolled with the group, and the social worker stated that consultant eye care consents were reviewed during quarterly care plan meetings and updated when new services were requested.
