During interview, the Nursing Secretary stated that the facility stopped putting the number of nursing hours and the resident census on the posting after beginning to send that information to corporate headquarters sometime in February 2026 and did not realize it was supposed to be posted daily. The DON and Administrator both stated that the nurse staffing information and resident census should be posted daily.

The Administrator stated the nurse staffing postings were posted the day before for the next day and typically revised within 24 hours as needed, but could not explain why the form did not have the facility's name or why actuals were not posted.
