The DON confirmed she accompanied the resident to the surgical center on 04/02/26 and 04/06/26, verified that the staffing tool reflected incorrect hours for her on 04/06/26, and acknowledged she was not in the facility continuously for eight hours and that there was no RN in the facility that day. The administrator and a regional RN also verified that the employment punches for 04/06/26 did not show the required eight RN hours for facility coverage.
