Summary

The deficiency involves the facility’s failure to store, prepare, and serve food in accordance with its own food storage, dating, and labeling policy and professional standards. The facility’s policy, last reviewed in April 2026, requires that food be stored in clean, dry areas free from contaminants, with adequate ventilation, illumination, and temperature/humidity controls. The policy also mandates proper stock rotation using the first-in, first-out method, dating and labeling of foods, maintaining refrigeration at or below 41°F, keeping freezers cold enough to keep food frozen solid, covering and dating all foods, and monitoring refrigerator and freezer temperatures at least twice daily with internal thermometers. Leftovers are to be covered, clearly labeled, dated, and used within five days or discarded. Surveyors observed a basement storage room that contained both food and nonfood items, including boxes of chips, candies, water, soda, and an unopened Amazon bag, with food stored in random areas rather than in an organized manner. In this same storage room, three opened honey mustard sauce containers were found on a shelf at room temperature, despite labeling that indicated they must be refrigerated after opening. Two of these sauces had a “best by” date of November 22, 2025, and one had a “best by” date of November 9, 2025. The storage room refrigerator was found packed with various food items, including fresh eggs, an open box of eggs, open salad dressings, pickles, whipped cream, open soda bottles, open mustard and ketchup bottles, a blue plastic container with a blue cover containing a grayish-brown unknown material, and two salted butter packages with a best-by date of September 17, 2024. Dried material was present on the bottom and shelves of the refrigerator. The top freezer compartment of this refrigerator was also packed with food items and lacked an internal thermometer. It contained two containers of ice cream and a box of fruit ice pops, with a blue bowl loosely covered with aluminum foil containing dried brownish material placed on top of the ice cream; this bowl was undated. Loose ice pops without dates were at the bottom of the freezer, and two large zip-lock bags containing undated frozen brown material were stored on top of the loose ice pops. Two packs of sausages were stored in the freezer door shelves, and the bottom of the freezer had a dried brownish-reddish substance. Interviews with the Business Office Manager revealed that she regularly conducts a fundraiser involving sausages, bread, chili, cheese, and sauerkraut, purchasing the food herself and storing it in this refrigerator when bought ahead of time. She stated that kitchen staff were not involved in the process, that Therapeutic Recreation and some nursing staff helped cook and serve during the event, that food temperatures were not checked at any time during storage or cooking, and that there were no temperature logs or documentation of refrigerator monitoring. The Director of Therapeutic Recreation confirmed that their department only helped cook and serve and was not involved in purchasing or storage, and the Dietary Director confirmed that the kitchen was not involved in storage or preparation for the fundraiser and that the Business Office Manager was responsible for purchasing, storing, preparing, cooking, selling, and serving the food. A Regional Nurse later confirmed the unsanitary and undated conditions in the refrigerator and freezer, including the absence of a thermometer in the freezer.
