A dependent, cognitively impaired resident with multiple comorbidities required assistance with ADLs and personal hygiene, including foot care. Over several bathing episodes, a CNA documented that the resident’s toenails were long, thick, and in need of podiatry services, and the primary nurse signed these shower sheets but did not arrange a podiatry consult or add the resident to the podiatry list.
